What is i‑IMPACT?

i‑IMPACT (Individualised self-Management ProgrAmme for long-term CondiTions) is an online self-management programme that is individualised for people with a long-term cardiac, respiratory or Covid condition.
This programme has been developed by a range of multidisciplinary rehabilitation healthcare specialists, wider experts and patients to manage their symptoms more effectively and reduce the impact of their condition on day-to-day life.
University Hospitals of Leicester Pulmonary and Cardiac Rehabilitation departments have over 10 years of experience in delivering web-based programmes such as Activate Your Heart, SPACE for COPD and Your Covid Recovery.
The research has shown the programmes to increase user’s knowledge around their condition, exercise performance and quality of life.

Features
Exercise
Following an individualised assessment with a healthcare professional, users will be encouraged to set goals and progress through a prescribed exercise programme tailored to their ability and symptoms.
Achieve weekly targets.
Record their activity and exercise to see improvements over time.
Gain advice on how to progress their exercise.
Trackers
The trackers allow a user to record and monitor some of the symptoms they may experience. Updating the tracker weekly enables the user to track when the symptom changed and how it has changed over the course of the programme.
It will sign post users to advice and support to help them self-manage their condition better.
Resources
The programme has many educational resources to help users understand their condition and some of the symptoms they may be experiencing.
Some of the topics are specific to their long-term condition for example; medications and disease education.
Some will include general advice on daily living such as; how to get moving, healthy eating and managing your day.
Ask a Healthcare Professional
There is a direct email messaging function via the programme through ‘Ask the Healthcare Professional’, which allows users to send questions to the local rehabilitation team.
Goals
Set a goal that is personal to the user. This can help users stay motivated and help them mange some of their symptoms. Use the resources available to help them achieve their goal.
